Intensive Service Plan Coordination

The Intensive Service Plan Coordinator provides leadership and support to the local service system to collaboratively develop Coordinated Service Plans that are the most effective for children, youth and their families, whose needs may exceed locally available services.
The Coordinator provides consultation to collaterals who are supporting children, youth and their families who may be supported through Children with Complex Special Needs funding.
The Coordinator provides case management support for children and youth who are currently supported through Children with Complex Special Needs funding and do not have the support of an organization that is a Coordinated Service Plan Provider.
